Brake pads are a wear item, none of them 'should' last forever.
Obviously, someone didn't inspect the brakes when rotating tires.
The inner rear pad is known to wear faster on the S60. I believe it has something to do with the EPB not releasing the caliper piston enough. Do not feel lonely, this has happened to very many Volvo owners.
