Fault code 0182 850

I've replaced the O2 sensor, cleaned the throttle-body, replaced the small hose that runs parallel to the PCV hose (it was ruptured) but I still get a "rich condition left bank" code in about 40 miles (after OBD II is cleared).

The car has 88k miles, and runs perfect. What else should I look at??

I'd guess it's possibly still a vacuum leak. Check volvospeed.com/bay13 for a list of common vacuum leak locations. A biggie is the elbow located out of sight down under the thermostat housing area.
