unwanted accelerator surge while braking S60 2003

I have a 2003 S60 base model with only 15,000+ miles.

Over the last year or so, after hard driving for an hour or so I occasionally get an accelerator surge even when I depress the brake pedal.

The car doesn't really lurch forward, but it does rock and move a few inches. I did not check the tachometer when this happens, but I am assuming the gas surge is reflected in the rpms. I had the car checked at dealer using computer diagnostics, but nothing showed up. The TCM has had issues in the past, but 2003 model year was supposed to have resolved this problem.

Any thoughts about this problem?
