Vibration 1st to 2nd gear V70-XC70 1998

Our 98 V70R has a bad vibration when accelerating from a stop. It only happens when the car is warmed up, and it only happens when it shifts from 1st to 2nd, no other gears or at speed. A prelude to the vibration is just when you press the accelerator, it will hesitate (no forward movement for a split second) slightly before "engaging" to take off.

Any thoughts?
