ALERT - Proximity of Gas and Brake Pedals S40-V40 2000

Has anyone experienced any problems with the close proximity of the accelarator (gas) and brake pedals in the V40?.

Recently while backing up into a parrallel parking space my V40 seemed to refuse to brake as I approached the car behind me. The engine wasn't racing but the force to move backward was at least equal to the braking I applied. It was truly a strange feeling not being able to stop the car. As a result I hit the SUV parked behind me and caused no damage to it but significant damage to my V40 rear lid and bumper. The only explanation I could determine for this freak accident is that as I turned to look over my right shoulder I had my foot equally upon the brake and accelerator pedals causing the car to continue backward. As I applied more pressure to what I thought was the brake I was equally accelerating.

I noticed that the two pedals are extremely close together the console resrticting my right leg coupled with the relatively close area for leg and foot room in the first place.

I would be interested in hearing from anyone who has experienced a similar incident. I THINK EVERONE SHOULD BE ALERT TO THIS POTENTIAL DANGER!!
