Use of shims on brake pads; rear speaker replacement on S70 850 1996

When replacing the brake pads, what is the proper method to determine the qty of shims to be used? What is the correct method of installation?

Also - my wife has a '98 S70 with a rear speaker that is blown. I took a look but find no easy way to remove the speaker. How does it come out? Any recommendations for a reasonably priced replacement?
