Two days ago, I noticed that I had no functioning headlights on my 99 V70. Worse yet, the car didn't know it didn't have functioning headlights (no indication on the dash). After some troubleshooting, I found that the headlight switch, which I never touch, had "exploded" from the back side. The rear of the switch had broken free after the plastic holes that hold the clips broke open. After extracting the switch, I was able to quickly repair with epoxy and, like magic, I had functioning headlights again.
The failure of this switch should be a critical "watch item" as it could potentially present a serious safety issue if it breaks at the wrong time. For those of you who have cars with the same switch, you might want to examine the switch to see if there are any signs of fractures where the back of the switch clips onto the front. If the back of the switch breaks away from the front even a fraction of an inch, you will lose your headlights!!!
