o2 Sensors,?? Physical condition of Faulty sensor ?

I have never replaced my o2 sensor onmy 1995 850 glt with over 120k miles, I am not getting codes, I was wondering if those of you who have replaced a faulty sensor could tell me what the physical condition was when you removed the sensor. Mine does not have carbon build up or soot on it. Also the holes are not clogged / blocked.
