Transmission problem? 850 1995

95 850 Turbo Wagon. 96,000miles. After driving at highway speeds for 30 minutes+ when I come to a stop my car tries to surge/pulse forward. If I then put it in reverse, it sounds like something is grinding as I back up. Sometimes when put in reverse, it doesn't find reverse at all until I first put it in drive. At the dealer, no codes for transmission were pulled. The fluid is old and advised to flush. No shavings or other debris in the fluid. thoughts?
