Other likely causes:
- engine coolant temperature sensor (ECT) not providing engine temperature information to ECU (so ECU doesn't enrichen the fuel mixture properly until engine warmed up). See Electrical: Engine Sensors, Diagnosing ECT failures in FAQ
- vacuum leek (so ECU thinks appropriate fuel/air mixture has been achieved, but amount of air getting into mixture is higher than what it is aware of). See Engine: Fuel Injection, Troubleshooting: Vacuum Leak Diagnosis in FAQ

So you don't mean cold as in frigid, right? Just not warmed up.

I had that symtom; car taking a very long time to start first time in the day, but only sometimes. Got progressively worse. I tried just about everything in my box of spares. It was the crank position sensor (RPM sensor) on the bell housing.

Don't know if that's your problem, but read the FAQ for more info on that part.
