When my car is cold, or sometimes warm, when it's under WOT, it almost stalls out and it caughs so to speak. Is this the cap and rotor needing replacement or what?

Are the ignition leads reasonably new? If they are the originals(!!) or not of the very best quality, the car will be better at large throttle openings with a new set of original equipment or performance leads. A very cheap performance upgrade if you drive hard.

Install new plugs while you are at it, and clean the coil and distributor caps of any surface dirt. Basic car stuff not unique to Volvos, but often overlooked.

Your poor car! WOT when cold! Smokeshows??? Tsk tsk.

The scenario you describe, cold, warm (not fully heated up)with WOT is a time when maximum fuel flow and pressure is needed. Filter and pumps and fuel pressure regulator must be in good shape. If you can hook up a pressure guage that would be the best way to check fuel pressures. I have even connected a guage and ran it through the hood seal and hung it on the outside mirror to observe while driving.
Of course ignition system must be up to snuff. That is basic before any diagnostics begin.

Have you checked your fuel pressure regulator? How about the base idle? have you checked for intake leaks after the turbo?
