Could be a number of things. I'll give you some ideas from my experience, starting with the most inexpensive first. I would first check simple things like vacuum leaks in hoses under the hood. If the throttle body has not been cleaned in ages, it's only the price of a gasket and will make a big difference in idle, performance and generally how smooth the car runs. More so, I would suspect the idle air control valve under the thottle body (large vacuum hoses in and out). Perhaps it is sticking until you get moving at bit. This generally, can also be cleaned (See FAQ section of brickboard for all the above). Then the question is if you fuel pressure regulator is OK. If you pull the vaccuum hose off and not change, thats a good indication, along with smell of gas seeping through the diaphram.
Next is whether you have good ignition. Good plugs and plug wires and no loose spark plugs.
Do you have turbo boost? If not, that is not going to be cheap. This is also the issue of injectors. I have never had to replace a volvo injector on any of my 4 volvos having over 200,000 miles on them. I simply buy the rebuild kit with new caps and o'rings to make sure that there is a good seal at the injector (not pulling air).
