uneven rear pad wear -- caliper or something else?

Hello,
I responded to some aweful scraping noises by changing my rear brake pads (and a rotor) to find that on one side, one pad was worn completely away (after only ~18k miles) whereas the other pad was barely used. Is this indicative of a failed caliper or might I just need new slide pins or something of the like?
