Crankshaft pulley keeps coming loose. HELP!! 90 740 Turbo 700

What am I doing wrong? How do I torque it down to stay? Or is it wearing out somehow? Any advice is greatly appreciated. I changed the timing belt and it's been coming loose ever since. Maybe I'm missing a lock washer or something?
