Hesitation and whatnot 200

I previously posted regarding my 1984 Volvo 240 and its inability to start regularly. It is starting better now, definitely worse when cold starting, but will always crank up after a few tries. It's also having a problem where it wants to hesitate and knock when accelerating hard from idle under no load. It was also sputter and hesitate under heavy load, but drives fine otherwise. I've checked the vaccuum routing, the air mass meter, and cleaned the throttle body.

Any ideas?
