2-3-1 code won't go away 200 1989

2-3-1 = compensating for lean or rich mixture at cruise.
I had this problem last year and it failed smog test. I finally put new cat and passed, but I don't think I really solved the problem. Now its back.

Runs smooth at cruise, rough at idle.
Plugs are clean so i think it is running lean.
Mileage is very consistent 10 Km/l = 23.5 mpg US, about 50% highway.

Over last year I have swapped out or replaced the following - no improvement (thank god for parts cars)
oxy sensor
crank sensor
fuel pumps (in-tank had died anyway)
FPR
dist cap + wires + plugs
air filter
fuel filter
intake bellows
intake manifold gasket
flame trap/oil separator
cannot find any vacuum leaks and TB is clean
no oil or water leaks
I even tried Italian tune-up

at idle(warm) it is missing on #4 cyl - found by unplugging injector.

I'm now thinking maybe low compression on #4, will test it next week.

I'm stumped - any ideas? Is it just getting old?
