'92 240 gentle surging 200 1992

I have a 1992 240 DL with 220K miles. It runs fine, but when driving at any kind of cruising speed -- 35 mph up to highway 65 mph -- the engine surges gently. I don't have a tach, so I can't say what kind of delta rpm I experience, but it surges in about a 5 sec. intervel. It idles fine and accelerates as well as any 240 with that many miles. I just feel something isn't right and some day I'll find myself completely stuck.
What could it be?
