rapid loss of coolant, no heat ... so what came first? 200

240 1989 wagon, had new radiator put in 6 months ago. Driving a couple days ago, I noticed the temp gauge needle going up towards the red. Then, next few miles, I noticed no heat coming through blower, and as well, the needle hit red. I stopped. Checked radiator and found "a hole" as if a 'vent plug' had blown off. Towed car to radiator mechanics, who said, yes, that hole should not be there! Will not hold coolant! So mechanic will replace radiator (under warranty) tomorrow ... my question is, what came first, the radiator breaking, or heater valve, or what? Any comments? I am wondering whose fault this is ... like did I get a cheap radiator put in, which will broke and caused damage to my engine ... or something else. Any comments?
