Does anyone recognise this halogen coversion for my 122s? 120-130

I broke one of the lens and I can't seem to locate the manufacturer. It says Bulb Type Sealed Beam on the front with that black C in the middle. Probably purchased in the 90s or before. Thanks. http://www.pbase.com/turborg/image/96303324
