tie rod worked loose from end 700 1992

fella bricksters:

about 9 months ago, I replaced the struts and front suspension bushings on abovementioned car. I had it professionally aligned after that job. what a difference all that made. About a month ago, my wife (this is her daily driver BTW) brought to my attention that the car again develop a bit of clunk in the front end, driver side, when hitting even small bumps, and also some vibration on the steering wheel at highway speed which today she said has gotten worse. I jacked up the car to investigate and switch the front wheels to the rear in hopes that it's just some imbalance on the front wheels. This is when I discovered that the passenger side tie rod had worked its way loose from the end (about 2 or 3 threads). I thought it was just the locking nut, but that was tight on the tie rod. This, more than likely has something to do with the steering vibration, but how about the clunk? What could have caused the tie rod to back out from the end? Hope to hear your input on this one fellas. Thanks.
