M46 tranny noise 700 1989

I just bought an '89 745Ti 4sp w/od. 120K.

I'm hearing a grinding/bearing noise in 1st through 3rd gear only when in gear and the clutch pedal is out. I don't hear it with 4th or od. It's tough to shift to 2nd from 1st but not from 3rd to 2nd. I looked at the FAQ and it seems that it might be the throw-out bearing or carrier bearing.

I'm looking for any help in diagnosing this.
