timing belt is stripped of teeth for the second time 200

The timing belt on my '83 wagon has stripped of teeth for the second time in the recent history.
I did not replace the belt tensioner, could that be it?

The first time this happened it was soon after I replaced the crank sprocket that broke in a oil seal job that went bad. I am going to replace the tensioner but would like to know if there could be any other causes. Did I do something wrong?
