Need help: shot seal at the back of the tranny.... 700 1987

HELP!

The seal between the tranny (automatic) and the driveshaft is shot (appears cracked and leaking). I can move the driveshaft by hand, up and down. Little bit of tranny fluid is leaking too. Does anyone know what is this seal called and how much is it? Part number maybe? Also, what is involved in R&R? Can I do it myself? I did replace motor mounts and rear tranny mount before by myself.
