Motor shakes 200 '86

I have an '86 240 DL. The car runs well except that motor shakes (left-right). It shakes worst when the car first starts, and smooths slightly as it warms up. The car always starts but always the second turn of the key. The first key turn just turns the engine over. The car get s decent mileage (22-24 mpg0, doesn't burn oil, and really gives me no other problem. I am concerned that this shaking can get worse or is a symptom of something serious. If you have seen this before let me know what you have done to correct it.

By the way, the car is tuned up, new fire trap, and throttle body service recently done. This helped the shake very very slightly.
