surging, hard starting, fuel problem? 200

My 1987 240 wagon has developed a choking, surging behavior. I've cleaned the throttle body as recommended, and even replaced a broken hose from the carbon cannister to the throttle body (the big one).Neither really helped with the problem.
Here are specific on the symptoms:
1) The car always starts up, but when cold dies almost immediately. It usually takes several tries to get it to idle more than a couple of seconds. We have to
rev it up to get it to go -- in other words, we can keep it from dying by giving it more gas.
2) It chokes and sputters when starting out -- again, it doesn't die, as long as we give it enough gas. Some backfiring.
3) Usually gets better as the engine warms up, and sometimes the problem seems to go away, but not always. Occasionally, it has diminished power even at
highway speeds, but never so badly that it drops below 55 mph. This is sometimes in the form of lurching.
4) The problem started abruptly after getting a fill up (bad gas?), but maybe that's just coincidence. I haven't changed the fuel filter recently, but if you think I should I will!

Does anyone have any more thoughts on what I should try next? Thanks in advance for your help!
