vacume leak 700

A friend of mine is thinking about getting this 740 Ti its got appx 150000mi. My question is what if any kind of damage does a good sized vacume leak cause? The guy thathas it has drivin the car with the vacume leak for a few hundred miles. He also says its getting really bad gas mileage. Is this due to the leak?
