My 93 245 won't idle until it warms up 200 1993

My trusty 93 245 has recently developed a problem in that it sometimes stalls after starting and won't idle. If I sit there and rev it up a bit for a few minutes, it runs fine. If instead I go ahead and drive it, it runs rough for the first few minutes and then runs fine. This happens infrequently, sometimes when it's cold and sometimes after a hot start. Any ideas are appreciated.
