Rough Idle -- but accelrates and runs great (1990 240) 200 1990

My 1990 240 Volvo has a rough idle (bad enough to shake the car). When I start the car it idles rough (a systematic and rapid change in RPMs--up down, up down, up down, etc.) and when it is warmed up it still idles rough. However, as soon as I increase the RPMs the rough idle goes away. The car accelerates smoothly and runs just fine, even at slow speeds (above idle RPMs). I have looked over my hoses and intake gasket. Any ideas?
