Clutch Pulsating; coming out of 1st and 2nd Gear; Clanking Sound 200 1990

I have a 1990 240DL with a 5 speed. I had been hearing a clanking sound around the clutch area for about a month, (just below my left foot) but I thought it was a loose exhaust pipe.

This weekend the clutch slipped in first and second gear, and hasn't been the same since. I can feel the clutch pull inward as I accelerate. There continues to be this rattling sound as I accelerate as well. Again, the rattling sound as I accelerate has been going in for about a month, but I thought it was simply an exhaust pipe issue.

I had my clutch re-built 2 years ago (20,000) miles.

Any idea what this could be?
