Timing belt slipped for the second time 700 1988

Has anyone ever had their timing belt slip twice? The first time it slipped, (Oct 31) it jumped at least 2 teeth and the car would barely run. Last night it slipped 1 tooth. The first time, it figured it was because the tensioner was loose, but this time, the tensioner was tight. Also, I didn't notice any problems with the belt (damaged teeth or anything).

I was just wondering 1 - if this is a common thing and 2 - what could be the root cause of this. Any ideas?
