Auto tranny doesn't always want to shift into 4th gear 200

I own a 93 240 Wagon with a little over 160K miles and had a couple of questions. 1) My transmission doesn't always want to shift into 4th gear. When this started happening I'd change the fuse for the OD and that would fix it. That happened maybe once a year. Now it happens often. When I get on the interstate and go high speeds, sometimes it takes forever to go into 4th (80mph until it finally dumps into 4th 4-5 miles later). 2) It took a while to get used to it when I bought the car, but I got used to it shifting hard. I remembered an old rule, "if you can feel it shift, it's less wear on the tranny." Now it's starting to shift really soft. That scares me. Is there any way other than a rebuild to make it go back to shifting hard? Is this just a sign of an older tranny going soft on me? Is there such thing as a tranny flush? I've had the fluid changed a few times and changed the tranny filter (I was told to be careful about doing if I don't have it changed regularly...may get trash in the fluid). Now what?
