High NOX failure, Need a second opinion 200 1992

I live in Fairfax VA,I have a 92 240 wagon w/ 167,000mi. We have one of the tightest DEQ tests in the nation, second I think to CA.I was stumped on my High NOX failure until I began reading the FAQ page, I think the high engine coolant temp caused the failure.
But here is what I have done,
set the vx3 cam timing to normal 0deg offset
new evap canister new vac lines, new intake billows
new idle air controller
set TPS correct
changed oil
new plugs
new catlitic converter new O2 sensor
new Thermostat
new coolant
new alternator
new alum radiator

then I failed!!!
test 1
HC ppm limit 108 reading 9 pass
CO% limit 0.60 reading 0.07 pass
NOX ppm limit 838 reading 931 FAILED!!!! WTF WTF
RPM 1667 15mph
test 2
HC ppm limit 104 reading 9 pass
CO% limit 0.58 reading 0.08 pass
NOX limti 759 rading 751 pass
RPM 1751 25mph

but Im running an electric fan, I think its not cooling enough
its a 16" fan, but Ive noticed it running a litle hot even at highway speeds when it should be cooling itself.. maybe it overheated? warped a head maybe??
BTW after changing all the above Im getting ~270-290mi per tank ~23mpg thats pretty damn good,

any suggestions..
