Erratic idle B23F 200 1984

I notice a slight surging of the engine, from 700 to 900 then back again, as I sit in drive at a stop light. If I just lightly hold the brake, it is enough to pull me forward for a second until it goes back down to 700. Intervals of perhaps 3 seconds each. If the car is in park (it's an auto), the tach stays steady, no surging.
I had earlier noticed what I thought was a very light surge at speed (say 50 mph) but it was so slight I attributed it to my imagination. Not so at idle. Anyone have any solution to this?
