upper hose extremely hot....overheating 200 1993

I am having some moderate overheating problems, after running for a few minutes the gauge jumps, the upper hose is extremely hot. What might the problem be. the upper hose should be cool for a stuck closed thermostat right??? DO water pump impellers fail???? Coolant level is fine. Cant get any heat out of the ducts in the cabin.
