87 Volvo High NOX???? 700

I just failed the TX emissions test with high NOX. I had the same problem last year and replaced the Catalytic converter and passed with flying colors. I can't imagine the cat being the problem again? Any suggestions? The car has 263,000 miles and oil is changed religiously at 3500 miles. Thanks! TPW
