ok, what's the connection between piston slap and exhaust leaks?? 200

Did some searches here trying to diagnose what I think is piston slap.I got a 82 with a b21 at about 190 thousand miles. no oil leaks, have never opened the head up... So far all I have learned is that:
1) it could be worn valve springs or retainers/hushers
2) could be an exhaust leak at the manifold (huh? how?)
or 3) could be piston slap as it reaches the apecies of the stroke
I do know that the noise is very pronounced on dead cold start ups and sort of goes away when the engine heats up. And it goea away completely when taking the car out of gear - its a m-46 - you can even rev it up out of gear and there is no noise, but once in gear pukity-puckity puckity. Please help me
