Alt. belt keeps coming loose 700

i was wondering if alt. belts coming loose is a problem with volvos. my g/f's works itself loose every few days. all the bolts are tight and it holds tension. do you think that maybe the bolt or the bracket is stripped? the belt has been replaced as well. do you know what size the bolt is? thanks
