14.3 Miles to the gallon... Woah S90-V90

Worked that out today.... No lights are on the dash or anything, always run premium. It just seems as if my gas mileage has gradually decreased... From 18 to 17 to 15 and now this. The car has new plugs. My endeavors to change the fuel filter were unsuccessful, and I don't believe it has ever been changed. The car has 85,000 miles. Do you suppose that could be the problem? Any other ideas? In all honesty, most of this tank was driven in E mode...
