Crankcase Ventilation / Oil Separator 200

The crankcase pressure on my 1989 240 (B23) got so high, it blew out the end cap of the valve (cam) cover.
The engine runs great, so excessive blowby is unlikely.
I cleaned the flame-trap; it didn’t look that bad.

I have not removed the oil separator (under intake manifold). Is this really an open "can" with strategically placed shields? - or is there some sort of filler in the separator that could be clogged?

Any other ideas what might have caused this excessive pressure?
