when clutches die... 200 1984

I have a question about what happens when clutches die. I always thought there would be slippage, i.e. the car doesn't go. The clutch on my 240 (M46+OD) has always been difficult to shift, you have to press all the way on the pedal (I mean all the way) to disengage it and shift. At one point I replaced the cable and it was great for a day or so, but I had to adjust it to the limit to even be able to shift the car. If I back off the adjustment any I can't shift the car at all even with the pedal all the way in. So it has finally reached the point where the cable is all adjusted all the way and the car is extremely difficult to shift.

To me this seems live reverse clutch failure. Is there a wearing point on the fork that could be causing this? Any thoughts?
