240 auto transmission only runs in first gear when cold 200 1992

The problem only occurs for a few minutes first thing in AM. I changed the oil, no difference. Once warm it will shift through all the gears. But when cold, it won't go out of first unless the selector is shifted to 2nd.

Any ideas on what the fix might cost? To save $$, what is the down side of trying to live with it until it gets worse? The transmission shop said they will check it out to see if it is external or internal? What could it possibly be external?
