Not a real problem having the hex on top in a 140. A short wobble extension should deal with the bolts easily -- I just happened to have broken mine. I'm sure it works fine on the bottom also -- maybe with a bit of grinding , as you say. Somehow it never crossed what passes for my mind to put it at the bottom.
