Remove the DIM and then provide the numbers that are on the back side, along with the year and model to Southern Vovo.

I did that and they were able to match it and sent me a perfectly functional used used unit for $115 including shipping. (Versus something like a $400 repair at Xemodex) The replacement works flawlessly. The biggest issue is that the replacement didn't have as many miles recorded as what my car actually had, so I've noted the delta value in my maintenance log and with a dymo label under he hood. (I'm not into fraudulently rolling back odometers.)
One caution though, and don't ask me how I know this - Don't start the engine while there is no DIM plugged in or it will record an SRS fault that you'll have to have cleared by a dealer! Or in my case it was a good independent shop that has Vadis capability - $30 and 10 minutes work. But since all the idiot lights have gone dead in your's, you may need to have that done after the R&R anyway.
