Help, Coolent is boiling 850

The other day I was driving my car and I started to notice steam comming out of my hood. It turned out that the cap had popped off from the pressure and water was gushing out of the reservor tank. Has done this every time the car warms up. Something is keeping the water from getting to the radiator or it is not cooling the the radiator. Need suggestions.(thermostat? waterpump? cooling fan?)
