Your description sounds more like a bad spring seat and not a bearing plate. If it is the spring seat you should be looking at 1 hour labor per side, $35.00 per spring seat and if you want to do the struts while you have them out it should be an additional $170.00 per side and I recommend aligning it at the same time. So to give you an idea it should look like this:

Spring seats installed: $210.00+tax
Struts $340.00+tax
4 wheel align $105.00+tax
Total $655.00+tax

This is with a shop labor rate of $70.00 per hour. Comes right in at about $700.00, huh?
