Can you give more details of the way his right front hit her car, and where it hit her car? How fast was she going? Was the van attempting to pull into her lane, or cross the road in front of her? (what angle did it occur at?) How did the roof get dented (from the impact?)? Side airbags deploy under specific conditions (like all airbags...)
