Randon bad mis-firing

Hi - Over the 2 months I have had randon (a few jerks)misfiring while driving on the highway. Last month we change the fuel filter (very plugged) and was good for about 1 week. It then started to randomly misfire again. Change filter again. (Lamda light on - code said lean fuel mixture) Good for 1 day. Now have random times when car feels like it runs only running on 1 or 2 cylnders for a minute, then runs great. Two minutes - later - mislfiring bad again. 4 minutes later - no problems for the next hour's drive.

Any ideas? Mas flow sensor? fiel pump?
