"I took a dental pick and really cleaned the grooves on the wiper arm shaft, then took a small triangular file and cut grooves inside the hole in the wiper arm, for a bit of "tooth". "

Sounds like you did the right thing. Cleaning the grooves is critical. I suspect that your earlier problems had resulted in the grooves being filled with aluminum shavings. Thus the nut wasn't really loosening but rather the aluminum of the new arm was being ground away leading to a loose fit.
