MAF / O2 Sensor Problems S70 1999

99 S70GLT with 161,000 miles:
MAF replaced at 124,000. Front O2 sensor replaced at 147,600. Rear O2 sensor replaced at 159,800 and less than a month later needed to be replaced again at 161,000 due to excessive carbon build up. Car still won't pass NJ inspection, now showing P0134 code (front sensor again?). Strong smell of unburned fuel, with excessive black exhaust. Trying to avoid the local dealers as previous experiences have not been great. Any thoughts on what might be going on?
