Chances are it is either a bad damper motor or a split shaft on the damper flap. There are two damper motors on the driver's side of the climate box, the temperature control motor is the upper one. Remove the lower dash panel for access. If the shaft is split I have seen it repaired by wrapping it with many loops of small gauge solid copper wire - Sort of the way the joints on a fishing rod are wrapped.
